Please print out and complete the following packet. You will need to have all paperwork at the time of your first appointment. If you are unable to print and complete this packet prior to your appointment, please arrive 30 minutes early to complete the paperwork in the office. If you fail to complete the packet prior to the scheduled time of your appointment, you may be rescheduled to ensure you are able to attend a full session.
If completing a packet for a minor, please ensure that both parents/legal guardians sign all signatures.
